Scabies

  •  Scabies is caused by infestation by Sarcoptes Scabies
  • The bug digs tunnels under the skin
  •  The rash is usually red bumps, knots or lines
  • The rash causes intense itching that is worse at night
  • Commonly affected skin locations include:
    • Interdigital web space
    • Wrists
    • Ankles
    • Breasts
    • Around the umbilicus
    • Genitals
  •  Immunosuppressed patient develop more severe form with thick whitish scale cover (Norwegian Scabies)
  • Transmission is caused by close personal contact
  • Diagnosis is made by examination only
  • Diagnosis may be confirmed by skin scraping examination under the microscope showing the mites, eggs or feces
  • Treatment:
    • Treat all members of the house hold
    • Wash and dry all bedding, linens, clothing and towels in high heat, repeat in 1 week
    •  Topical 5% Permethrin cream applied from neck down overnight, wash off in am, repeat in 1 week
    • Ensure the cream is applied under finger nails after thorough cleaning

Lice

There are 3 types of lice that infest humans:
  • Head Lice (Pediculus Humanus Capitals)
  • Body Lice (Pediculus Humanus Corporis)
  • Pubic Lice (Pthirus Pubis)
Head Lice lives on the surface of the scalp and attach eggs (nits) to proximal hair shaft. Head Lice transmission occurs by close personal contact or through bedding, clothing or hair brushes

Body Lice live in these seams of clothing and not on the skin The rash is usually excoriation resulting in crusted bumps and red lines (skin bleed)
Macular Ceruleae is blue-brown spots caused by bleeding under the skin from the bug sucking blood for feeding Usual location is neck, trunk and proximal arms

Pubic Lice is usually transmitted by sexual contact and my be transmitted to other sites like eyebrows or eyelashes
Treatment:
  • 1% Permethrin cream rinse, 0.5% Malathion for resistance
  • Clean nits with fine tooth comb
  • Wash and dry clothes and beddings in high heat
  • Use Petrolatum ointment for eyelash infestation
